Save time at orientation by downloading and pre-filling your child’s registration forms at home. Bring the completed packet with you to orientation so we can review and answer questions on teh spot. If you’re unable to print, we’ll have copies available at school.

NOTE: One packet per child enrolled.

Quick Checklist

Before you start to fill out the forms. Please make sure you have the following items readily available:

• • • Government issued photo ID for the adult completing the forms
    • Your child’s health insurance info and pediatrician contact
    • Emergency contact names and phone numbers (non-parent & guardian)
    • Any allergies/medical notes and daily medications
    • Pick-up list (names of adults who may pick up your child)

How It Works

It’s a super simple 3 step process!

1. 1. Download the forms packet below for your child.
   2. Prefill on your device or print and complete in pen.
   3. Bring the completed packet to orientation. That’s it!
